Admission
requests are called in by your physician - your physician
will provide the The following information:
- Patient name
- Patient age
- Patient phone number
- Type of bed required
- Reason for admission (diagnosis)
Physician
office is responsible for contacting your insurance company
for authorization and/or pre-certification - if required by
your insurance.
Inpatient
registration (Admitting) office will contact you with a room
assignment and arrival time.
If you do not require Pre-Admission Testing (PAT) you will
arrive at the Admitting office and need to bring the following:
If
admission is not related to a Workmen's compensation injury
and not related to a No-Fault accident (Auto accident):
- Present your insurance card(s)
- Present your personal identification
- When possible, please leave your valuables at home
If related to a Workmen's compensation injury:
- Have your Workmen's compensation carrier or employer name
- Have your Workmen's compensation carrier or employer address
- Policy and/or claim number
- Adjuster name
- When possible, please leave your valuables at home
If
related to a No-Fault accident (Auto accident):
- No-Fault carrier
- No-Fault address
- No-fault policy/claim number
- Adjuster name
- Present your health insurance card(s) - to bill after
No-Fault
- Present your personal identification
When possible, please leave your valuables at home.
